3. Documents required for granting the Certificate of Right to Medical Assistance - CDAM
3.01 - Copy of round-trip tickets to the destination where the CDAM is being requested. (NOTE: For Citizens who need the CDAM to present at Consulates or Universities, the Services and Divisions responsible can issue the Certificate and communicate to the user, as soon as the tickets are issued, the beneficiary must deliver a copy for registration. user does not return with the copy, the pending issue remains in the system. When you need the issuance of the next one or an extension, only issue with the regularization of the previous pending.)
3.02 - For employees with a signed Work Permit and their dependents
3.02.1 - Copy of CPF (Individual Registration)
3.02.2 - Updated Proof of Residence
3.02.3 - Copy of Passport
3.02.4 - Copy of the Employment Card (qualification and employment contract), with the date of affiliation to the General Social Security System; (Article 28 of Presidential Decree No. 3048 of May 6, 1999)
3.02.5 - Dependents who will travel:
a) Copy of Passport;
b) Copy of the Marriage Certificate, if applicable;
c) Copy of the Birth Certificate of children under 21 (twenty-one) years of age.
 

3.03 - For self-employed workers, who pay the INSS
3.03.1 - Copy of CPF (Individual Registration)
3.03.2 - Updated Proof of Residence
3.03.3 - for the insured domestic employee, individual taxpayer, subject to the provisions of § 4 of art. 26, special, the latter as an individual taxpayer pursuant to the provisions of § 2 of art. 200, and optional, from the date of the effective payment of the first contribution without delay, not being considered for this purpose the contributions collected with delay referring to previous competences, observing, as for the optional insured, the provisions of §§ 3 and 4 of art. 11. (Article 28 of Presidential Decree No. 3048 of May 6, 1999)
3.03.4 - Copy of Passport
3.03.5. - Dependents who will travel (ditto sub-item 3.02.4).
 

Art.28. The grace period is counted:
I - for the insured employee and independent worker, from the date of affiliation to the General Social Security System; and

II - for the insured domestic employee, individual taxpayer, in compliance with the provisions of § 4 of art. 26, special, the latter as an individual taxpayer pursuant to the provisions of § 2 of art. 200, and optional, from the date of the effective payment of the first contribution without delay, not being considered for this purpose the contributions collected with delay referring to previous competences, observing, as for the optional insured, the provisions of §§ 3 and 4 of art. 11.

3.05 - Retirees and their dependents
3.05.1 - Copy of CPF (Individual Registration)
3.05.2 - Benefit number;
3.05.3 - Copy of Passport;
3.05.4 - Updated Proof of Residence;
3.05.5 - Dependents who will travel (ditto sub-item 3.02.4).
a) The documentation must be presented with authentication at a Notary, Embassy or Consulate. If you prefer, present the originals so that the copies are authenticated by the issuing body of the CDAM (Ministry of Health).
b) The interested party must, upon delivery of the documentation, provide the address of residence and domicile of the insured in Brazil and in the country of destination.
c) It is forbidden to supply a certificate to public servants governed under the aegis of Law 8.112/90 for the countries: Italy and Cape Verde.
d) The certificate will be valid for one year from the date of signature.
Note: The validity of the document is 1 (one) year, renewable, from the date of issue or future date of travel tickets. The document can be delivered within 03 days of the request. It is important to note that the Certificate is not equivalent to travel insurance.
